To register as a patient at Brook Lane Surgery you will need to bring your medical card to reception. If you have mislaid your medical card there is a form that can be completed instead. It is advisable to telephone the surgery prior to registering to ascertain that your new address falls within the practice boundary and that we are currently accepting new patients.
