OBTAIN
A REPEAT PRESCRIPTION?
Computer Printouts
If
your doctor has agreed to you having regular prescriptions they will have arranged
for you to have a computer printout showing your personal number and the medications
you are to take. There are several ways of requesting a repeat prescription using
this printout.
- Please telephone 01489 574063 between
10.30am - 12.30pm. Please have your computer printout number ready to give to
the receptionist.
- You can bring your computer printout to the surgery,
clearly indicating the items you require, and leave it in the box provided.

You can post your printout to us, again remembering to indicate the items you
require. Provided you enclose a stamped, self-addressed envelope, we will post
your prescription back to you.
- Your printout can be faxed to us on 01489
570033 clearly indicating the items you require.
If you require a repeat prescription please
submit your request in writing, stating your name, address and full details of
the items you require.
As our receptionists are not medically qualified
they are not allowed to accept prescription requests over the telephone.
Your
doctor may ask to see you before issuing another prescription - this is to ensure
that you continue to receive the treatment best suited to you.
Please
allow 48 hours' notice for all repeat prescription requests.

A SICK CERTIFICATE?
"Self-certification" is the procedure
you use if you are ill and therefore unable to work for up to seven days. These
forms are available from your employer or the post office. After this period of
time, if you are still too ill to return to work, you need to see a doctor to
get a certificate for your employer. If you are self-employed the necessary forms
are available at the surgery.
