In accordance with the Data Protection Act 2018 and Access to Health Records Act, patients may request to see their medical records. Such requests should be made through the practice and may be subject to an administration charge. No information will be released without the patient consent unless we are legally obliged to do so.
Request a copy of your health record
You are legally entitled to request a copy of your medical record and depending on the information you require and the nature of the request you can do this in one of two ways:
- Request a Brief Summary of your records
- Submit a Subject Access Request (SAR)
Request a Brief Summary
You can request a brief summary of your medical records, in person at the surgery. Please note you will need to bring photographic ID for verification purposes and identification.
The information in a brief summary is the same information available to you through the NHS App and we encourage all patients to access their information this way.
If you require information that is not available through the NHS App, or a brief summary, then you will need to submit a Subject Access Request with the practice.
Subject Access Requests (SARS)
Subject Access Requests can be submitted with the practice, where information is required which is not included in a brief summary of your medical records. Please be assured we follow strict guidance on data protection and confidentiality to ensure copies of medical records are provided to the correct individual, while also ensuring only the requested information is shared from your medical record.
If you wish for someone else to request this information on your behalf (relative, friend, solicitor etc) we will require explicit signed consent before the request is actioned. Please note, SAR requests have a legal turnaround of 30 calendar days, starting from the date they are submitted with the practice, and pending any prerequisite information. In the event information has not been provided to support the request any applicable timescales will not apply until this has been received. In addition, where requests are complex and require further review, we will formally write to you to request an extension.
