You must be aged 13 or over and registered at a GP surgery in England.

How we can help
Some people will find that they can setup and use the NHS App without needing to visit us. If you are presented with a screen that won't link your app to us then please visit the surgery. You will need to complete our online access request form and show us photo ID, if you have one to prove it is you.
We can also help you with the setup of the app and show you how to use it if you are having issues. Please visit the surgery and ask us for assistance. Please bring the device with you such as your phone or tablet so that we can guide you through the setup.
